Entry Form:

2. Print the form on your computer.
3. Complete the registration form on the 2nd page.
4. Mail your completed entry form & registration fees to the address at the bottom of the form. (Your entry form postmark must be no later than 4/24/2010 to take advantage of the reduced entry fee and be assured of receiving a BBQ ticket, tournament hat and tournament towel.)
